Please complete the client referral form and return a signed service agreement before your initial assessment.
An initial music therapy assessment is completed before ongoing therapy begins. This allows the Music Therapist to understand the client’s individual strengths, needs, interests and goals, and to determine how music therapy can best support them.
The assessment may be completed in one session. However, where additional observation or assessment is required to gain a more comprehensive understanding of the client’s needs and to support appropriate therapy planning, further assessment sessions may be recommended. Assessment will generally not exceed three sessions.
Following the assessment, the Music Therapist will develop appropriate music therapy goals and recommendations to guide ongoing support.

Assessment Report Feedback
$104.10 for up to a 40-minute session

Progress Report
$468.48 non-client contact time
(up to 3 hours, then prorated at hourly rate)
Travel Fee (Additional)
Where travel is required to provide a face-to-face service, Voices of Heaven™ may charge provider travel in accordance with the current NDIS Pricing Arrangements and Price Limits.
Provider travel may include travel time and reasonable non-labour travel costs, such as vehicle kilometres, where permitted under the participant’s NDIS funding arrangements. Applicable travel charges will be discussed and agreed upon before services commence.
